Okay, jetzt wird es schwer fachlich - und ich betone, dass meine Angaben keine Verbindlichkeit besitzen:
Der Standard ist die RPE MIT NE (nerverhalt), das ist international akzeptierter Konsens.
Begründung: Erektionsfähigkeit und damit Lebensqualität (klar), aber auch -und dies hat für den älteren RPE-Patienten mit (beg.)ED hohe Relevanz- bessere Kontinenzraten bei NE, wobei nicht endgültig geklärt ist, ob dies auf bessere Erhaltung der anatomischen Integrität oder auf bessere nervale Funktion zurückführbar ist.
Die Indikation zur Nerventfernung ist aktuell UNABHÄNGIG von ED oder Kontinenz(die ja präop selten nicht besteht) zu sehen, ist also abhängig von Gleason, PSA, Karzinomverteilung in der Stanzbiopsie und Tastbefund/TRUS - und manchmal auch vom intraop. Befund.
Damit ist die Entscheidung durchaus sehr multifaktoriell zu sehen, ist jedoch für mich eigentlich zu 95% immer nachvollziehbar bzw. meine Aufklärung entspricht der endgültigen OP-Indikation.
Wie wird es bei euch gehandhabt, was sind cut-off-Werte?
