"The seventh question is whether it is possible to make a claim of noninferiority if your trial did not demonstrate superiority. The VALIANT trial[14] is interesting in this respect. It tested valsartan (a new drug at the time) versus captopril (or both) in complicated MI patients. The primary outcome was all-cause death. They demonstrated complete neutrality when it came to an HR of 1 for valsartan versus captopril—obviously, not significant. The confidence interval (CI) excluded a predefined noninferiority margin of 1.13, and therefore they were able to make a claim that there is evidence that valsartan is noninferior to captopril, which may not seem very exciting, but because many patients have side effects on captopril such as cough, valsartan may be a useful back-up therapy, and this trial is valuable in demonstrating that."
